Q: I Want to Get a Facelift. I Had Sculptra Last Year Will That Be a Problem?

A: Face lift and fillers

To answer your question simply, it should not be a problem. Ideally I love to do the face lift first then assess volume issues later the practical reality is that as common as fillers are, we do facelift in filler patients all the time.

Many patients who want to hold off or even avoid a facelift use fillers as a substitute. However, I always tell patients to remember some basic logical principles. Fillers simply add volume, so they are best utilized where volume loss occurs. That usually occurs in the temples and cheeks as we age.

The reason that smile lines get more pronounced is by gravity causing the cheek to fold over the upper lip and mouth area.Smile lines are not caused by having volume loss limited to this area.  Placing filler in this area will certainly lessen the line but causes more volume in this area which has never been there before. Just know that if you change areas instead of restore areas you will, by definition, look differently. That may or may not be desirable but you must consider it, at least. If small volumes are placed subtly then a desired improvement can be achieved, but at some point the smile and animation movements in the cheek begin to look “different”. Friends and family can pick up these changes and if severe, anyone can tell. I say this only to caution other readers to “be careful” about overuse of fillers.

I prefer to do a facelift and get the structures returned to the appropriate natural position, then if the volume loss that occurs with aging is significan’t then it can be addressed by fillers in the cheek and temple. This gives a natural, youthful appearance.

Good luck with your surgery. The filler issue can be overcome. I would encourage you to find a good board certified surgeon that you can trust and communicate with.

Optimal Skin Care Regimen from Dr. Burns

To my patients,

 

I believe the quality of your skin may be the single biggest factor to maintaining a youthful appearance. There is no doubt that surgical correction is necessary to achieve optimal facial rejuvenation. However, optimizing the quality and health of your skin enhances surgical results and ensures that your skin appearance matches the overall post surgical rejuvenation appearance.

Many of you have asked me my “optimal skin care regimen”. Although there is no one regimen right for everyone, there are some basic features that I believe are valuable to almost everyone.

 

  • Sunscreen UVA/UVB
    • Everyone should utilize a sunscreen daily with Zinc oxide. It provides a full block to harmful rays
  • TNS Essential Serum (topical nouricell serum)
      • There is simply nothing like it on the market. The product contains TNS, which contains over 90 % of human epidermal growth factors that produce more youthful collagen within the dermis when applied.. Our practice was involved with the initial studies in proving its effectiveness many years ago. It stands as the #1 product in our office due to the science behind it, as well as extremely high patient satisfaction.
      • Although TNS can be purchased alone, the Essential serum contains peptides and powerful antioxidants that fight and block the harmful cancer causing “by- products “of UV rays.
  • Lytera
    • Our practice was involved firsthand in clinical trials that proved that this NON- HYDROQUINONE product was equal to the Obagi Nu derm gold standard for problematic dyschromia (brown spots and sun spots, blotchiness). It attacks all 4 melanin producing pathways. No other product currently can make that claim. It smooths out the color of the skin and gives it a youthful glow that adds greatly to the overall skin health and appearance.
  • Retinol Complex
    • Retin-A is a wonderful and effective product that causes skin cell turnover and more youthful skin, but is not tolerated by most patients due the intensity of the peeling and dryness it causes. Retinol Complex and Retinol Complex ES (extra strength) are excellent alternatives that produce similar results to Retin- A, but are much better tolerated.

Q: How Long Does It Take to See Final Liposuction Result?

A: How long until you can evaluate the final liposuction results

The answer here is fairly straightforward. You can rely on a final result of liposuction in most cases at 6 months post operatively.

If you saw results immediately and then saw those results decline, you are either seeing swelling(if early in the first 6 weeks) or you are gaining weight. However, if you gain weight after liposuction you do not typically see it in the areas that have been liposuctioned. You see weight gain in the areas that were not suctioned.

I hope this helps you sort things out, but be as patient as you can for the first few months.

Dr. Burns chosen by peers as one of the “Best Doctors in Dallas 2012” in D Magazine





How does D Magazine determine which doctors make the list?
Best Doctors is a peer-review voting process. We rely on the doctors’ expertise to determine who deserves to be on the list, just like a doctor would recommend a patient to a specialist. This year, we mailed a letter to 7,913 local doctors from our online directory inviting them to vote using a ballot on our website. They could vote for up to three doctors in 43 categories. The nomination form asked them to cast a vote bearing in mind the following question: which Dallas doctors would you trust with the care of a loved one? This year, 2,012 doctors voted. We vetted each voter’s Texas medical license number to ensure they were valid. We then sent the results to an outside marketing firm to tally the votes. We presented the tallied list to an anonymous panel of esteemed physicians. The panel and D staff reviewed the tallied results and decided the finalists based on the number of votes in each category. Once the list was finalized, D confirmed each doctor’s Texas medical license status and board certification and investigated his disciplinary status.

Questions & Answers with Dr. Burns

Question:

I weigh 132lbs., am  5′ 1″ , age 52. I have been trying to lose 5lbs before my TT and Lipo (Flanks and lower back) surgery . Is this weight reasonable for surgery? I really would like to be 122lbs but am having a hard time losing it. Also I wear a size 10 -approximately how many sizes will I go down after surgery, if any?  I am so scared and nervous as this is kind of a taboo in my culture to have cosmetic surgery , so I am not going to be able to tell anyone except hubby and kids. Thank you.


Answer:

Not to sound too simplistic, but the the ideal weight for any operation is your ideal body weight and there are charts everywhere that can tell you that. However the practical issues are that very few people are actually at their ideal body weight. So you are asking a great question. What is realistic from a real life perspective?

So for any tightening procedures such as tummy tuck, face lift, breast reduction, or breast lift, the closer you are to your ideal weight the the better it is theoretically. Less fat and more skin  excess means simply that you can excise more of the excess skin. If you gain a little weight afterward. then it will only tighten the skin to some degree and not grossly distort the result.

Therefore in your case, you are at a very reasonable weight for your surgery, but I always tell my patients that if you want to lose any amount of weight prior to surgery, then I am all for it, unless the weight loss would go beyond the ideal weight.

The only exception to this rule is for liposuction. For liposuction, I believe patients should be at the weight they can maintain. If you lose weight lower than what you can reasonably maintain long term then when you gain your weight back, it will go to unusual places. This unusual fat redistribution after liposuction causes people concern, but it will NEVER happen if you do not gain weight after liposuction. So enter liposuction at a weight that you can maintain easily.
